Hi,

It's not come from our theme - it come from plugins you're using.

Please try deactivate all plugins ( except plugins come from the theme ) and try it again. If it works, let's enable each plugin to check what's plugin caused this issue. 

Note: Because you've logged in with administrator account - so you won't see the "Name", "Email", "Website" fields. You need to logout to see that fields.
