yes this is the code i use and this is the message i get when try to subscribe with widget on front page

Have you use this markup we've provide here?
on this line:

doesn't exists required code

For mailchimp I am using the code form theme documentation section.
I cannot join mailing list with footer signup form widget.
When using full form od a sub page it works but with widget not.
is this because there is name filed in the code but not in the widget?
why there is no name in the widget?
how to add name to widget?
